To obtain a replacement (duplicate) title, the first step is to fill out a Title and Registration Application form available at the link https://azdot.gov/mvd/forms-library Check “Duplicate” box at the top of the form. Only one owner has to sign it.
Take it to a MVD office. Locations can be found at this official link from the Azdot.gov

You can mail it to Motor Vehicle Division, PO Box 2100, MD 555M, Phoenix, AZ 85001. Fee is $4. If replacing a damaged title, the damaged title must be attached to the application. If the title showed a lienholder (lender), you must submit a document showing the lien is paid off. Otherwise, the title will be sent to the lienholder.

REPLACEMENT TITLE – ARIZONA
You cannot transfer ownership of your vehicle if you do not possess the Title Certificate. The most common reason you would not possess the Certificate is that there is a lienholder (lender) for the vehicle. In this case the lien holder maintains control of the title until the lien (loan) is paid off. This common situation does not require a replacement of the original title, but instead a transfer of the title to you.
REPLACEMENT. The title must be “replaced” if the original has been lost, damaged (rips, coffee stains, water damage), has cross outs or white outs, or has incorrect entries (such as seller’s printed name and signature not matching the official names on the front of the title) or signatures in the wrong place.
