The required application form for a replacement (duplicate) title is available at the link https://www.ok.gov/tax/documents/701-7.pdf Complete the form and have your signature NOTARIZED.
By mail. Send the form and $12.50 to the address on the bottom of the form (The Oklahoma Motor Vehicle Division suggests on the form that you would be better off to take the form in person to an Oklahoma motor license agent).
In person. Bring the form to your lcal motor license agent. Locations can be found at https://www.ok.gov/tax/Individuals/Motor_Vehicle/Find_a_Tag_Agent/

REPLACEMENT TITLE – OKLAHOMA
You cannot transfer ownership of your vehicle if you do not possess the Title Certificate. In Oklahoma the registered owner of the vehicle is the one who maintains possession of the paper title.
REPLACEMENT. The title must be “replaced” if the original has been lost, damaged (rips, coffee stains, water damage), has cross outs or white outs, or has incorrect entries (such as seller’s printed name and signature not matching the official names on the front of the title) or signatures in the wrong place.
