The required application form for a replacement (duplicate) title is available at the link https://dps.mn.gov/divisions/dvs/forms-documents/Documents/MV_DuplicateTitle_Reg_Cab_LienCard_Application.pdf. Fill in the lienholder (lender) if applicable. If your loan has been paid off, have the lienholder sign and NOTARIZE, so you can be issued a clear title. If replacing a damaged title, attached it to the form.
By Mail. Send the completed application form (and damaged title, if applicable) and $17.25 (payable to Driver and Vehicle Services) to: Driver and Vehicle Services – Central Office, 445 Minnesota Street – Suite 187, Saint Paul, MN 55101-5187.
In Person. Bring the form to a Deputy Registrar Office. Locations are shown at https://dps.mn.gov/divisions/dvs/locations/Pages/find-office-locations.aspx Under “Title and Registration” choose either Vehicle Titling and Registration… or Vehicle Title-Expedited. Offices that can do Expedited service will charge an additional $20, but they guarantee processing within 3 business days or less. The Central Office noted in the mailing address above can issue duplicate titles on the next business day.

REPLACEMENT TITLE – MINNESOTA
You cannot transfer ownership of your vehicle if you do not possess the Title Certificate. In Minnesota the registered owner of the vehicle is the one who maintains possession of the paper title.
REPLACEMENT. The title must be “replaced” if the original has been lost, damaged (rips, coffee stains, water damage), has cross outs or white outs, or has incorrect entries (such as seller’s printed name and signature not matching the official names on the front of the title) or signatures in the wrong place.
