By Mail. Send to Vehicle Services – Title Section, Bureau of Motor Vehicles, 29 State House Station, Augusta, ME 04333-0029, NH 03305. Include check or money order for $ 33 payable to “Secretary of State”. Or, check RUSH REQUEST at top of form and send $43.

In person. Bring the form (or the info needed for the form) to a Bureau of Motor Vehicle Office. Locations can be found at http://www.maine.gov/sos/bmv/locations/index.html
