The required application form for a replacement (duplicate) title is available at the link http://transportation.ky.gov/Organizational-Resources/Forms/TC%2096-182.pdf At the top of the form check the “Duplicate” box, and then check one of the boxes below that. Then fill in the Vehicle Identification Section. When you apply you will also need the title number and/or the license plate number. Down and to the right fill in your owner information at the OWNER/BUYER lines. Do not sign without a Notary present, or at the County Clerks office.
Bring the application and your photo ID to your County Clerks office. Locations are given at http://drive.ky.gov/Pages/County-clerks.aspx. The fee is $6.00, but you can have done quickly for a $25.00 fee. There may also be a Notary fee.

You cannot transfer ownership of your vehicle if you do not possess the Title Certificate. In Kentucky the registered owner of the vehicle is the one who maintains possession of the paper title.
REPLACEMENT. The title must be “replaced” if the original has been lost, damaged (rips, coffee stains, water damage), has cross outs or white outs, or has incorrect entries (such as seller’s printed name and signature not matching the official names on the front of the title) or signatures in the wrong place.
